All health professionals should have expertise in palliative care and know when to refer to specialist palliative care to assist with complex situations.<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section elementor-element elementor-element-aaac07d elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"aaac07d\" data-element_type=\"section\">\n\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"elementor-column elementor-col-100 elementor-top-column elementor-element elementor-element-0e6c83c\" data-id=\"0e6c83c\" data-element_type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-4326064 elementor-widget elementor-widget-text-editor\" data-id=\"4326064\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<h4 style=\"font-size: 1.42857rem; font-style: normal;\"><span style=\"font-size: 20px; color: #d10f0f;\">When should I consider care for a patient?<\/span><\/h4>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e5f64b7 elementor-widget elementor-widget-text-editor\" data-id=\"e5f64b7\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><span style=\"color: #000000;\">Palliative care can begin from the first diagnosis of a life limiting illness. It can be provided side by side with treatment to manage complex pain and other physical and psycho-social symptoms with the goals of increased patient comfort and improved quality of life for the patient and their family. There are also clinical triggers for referral to palliative care for chronic conditions such as poor symptom control, frequent exacerbations and\/or admissions to hospital.<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section elementor-element elementor-element-2b1ad60 elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"2b1ad60\" data-element_type=\"section\">\n\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"elementor-column elementor-col-100 elementor-top-column elementor-element elementor-element-875f0d0\" data-id=\"875f0d0\" data-element_type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-45822fc elementor-widget elementor-widget-text-editor\" data-id=\"45822fc\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<h4 style=\"font-size: 1.42857rem; font-style: normal;\"><span style=\"font-size: 20px; color: #d10f0f;\">Role of Health Professionals<\/span><\/h4>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-231ba0c elementor-widget elementor-widget-text-editor\" data-id=\"231ba0c\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<h5><span style=\"color: #000000;\"><strong>General practitioner (GP) or family doctor<\/strong><\/span><\/h5><div data-height=\"322\"><ul><li><span style=\"color: #000000;\">continue to see patient for day-to-day health care issues if patient is being cared for at home (and may make home visits)<\/span><\/li><li><span style=\"color: #000000;\">liaises with the nurse and\/or palliative care specialist about the coordination of patient&#8217;s ongoing care<\/span><\/li><li><span style=\"color: #000000;\">refers patient to a palliative care specialist<\/span><\/li><li><span style=\"color: #000000;\">can organize admission to hospital or a palliative care unit if patient&#8217;s circumstances change<\/span><\/li><li><span style=\"color: #000000;\">offers support to the patients, their family and carers<\/span><\/li><li><span style=\"color: #000000;\">can provide referrals for counselling, including bereavement counselling if necessary<\/span><\/li><\/ul><h5><span style=\"color: #000000;\"><strong>Nurse<\/strong><\/span><\/h5><div data-height=\"370\"><ul><li><span style=\"color: #000000;\">may be a community nurse, a specialist palliative care nurse or a palliative care nurse practitioner<\/span><\/li><li><span style=\"color: #000000;\">may work for a hospital, community nursing service, residential aged care facility or specialist palliative care service<\/span><\/li><li><span style=\"color: #000000;\">if patients are being cared for at home, may visit them in their home and may provide after hours telephone support<\/span><\/li><li><span style=\"color: #000000;\">coordinates other health professionals and works out what care patients need, including home nursing or personal care assistance<\/span><\/li><li><span style=\"color: #000000;\">makes sure patients have access to medicines and other treatments for pain and symptom relief, and talks to patients about how to take their medicines<\/span><\/li><li><span style=\"color: #000000;\">can suggest practical strategies to help patients manage their condition (e.g. how to manage fatigue or loss of appetite)<\/span><\/li><\/ul><h5><span style=\"color: #000000;\"><strong>Palliative care specialist or physician<\/strong><\/span><\/h5><div data-height=\"339\"><ul><li><span style=\"color: #000000;\">prescribes or recommends treatment for pain, nausea, constipation, anxiety, depression, shortness of breath or any other symptoms that the patients may have<\/span><\/li><li><span style=\"color: #000000;\">usually provides care in a palliative care unit or hospital (both for inpatients or people attending an outpatient clinic), but may also be able to visit patients in their home or residential aged care facility<\/span><\/li><li><span style=\"color: #000000;\">communicates with and advises the cancer specialist and patients&#8217; GP to ensure that the treatment is well coordinated<\/span><\/li><li><span style=\"color: #000000;\">may refer patients and their families to a grief counsellor, psychologist or other support person<\/span><\/li><li><span style=\"color: #000000;\">assists with decision-making about care and treatment choices<\/span><\/li><\/ul><h5><span style=\"color: #000000;\"><strong>Cancer specialist<\/strong><\/span><\/h5><div data-height=\"260\"><ul><li><span style=\"color: #000000;\">may be a medical oncologist, surgeon, radiation oncologist or haematologist<\/span><\/li><li><span style=\"color: #000000;\">diagnoses the advanced cancer and may refer patients to a specialist palliative care team<\/span><\/li><li><span style=\"color: #000000;\">continues to oversee treatment aimed at managing symptoms of the cancer (such as\u00a0<a style=\"color: #000000;\" href=\"https:\/\/www.cancercouncil.com.au\/cancer-information\/advanced-cancer\/palliative-care\/treatment\/common-treatments\/\">surgery, chemotherapy, targeted therapy or radiation therapy<\/a>)<\/span><\/li><li><span style=\"color: #000000;\">may manage some aspects of palliative care<\/span><\/li><\/ul><h5><span style=\"color: #000000;\"><strong>Counsellor or psychologist<\/strong><\/span><\/h5><div data-height=\"408\"><ul><li><span style=\"color: #000000;\">trained in listening and counselling<\/span><\/li><li><span style=\"color: #000000;\">allows patients and their carers to talk about any fears, worries or conflicting emotions that they may be feeling<\/span><\/li><li><span style=\"color: #000000;\">helps patients and their carers identify and talk about feelings of loss or grief<\/span><\/li><li><span style=\"color: #000000;\">assists patients and their carers to communicate and to explore relationship or emotional issues<\/span><\/li><li><span style=\"color: #000000;\">might suggest strategies, techniques and ways of lessening the distress, anxiety or sadness patients and others are feeling<\/span><\/li><li><span style=\"color: #000000;\">helps patients and their carers to explore the issues you are facing so you can find more pleasure in your life<\/span><\/li><li><span style=\"color: #000000;\">may show patients meditation or relaxation exercises to help ease physical and emotional pain<\/span><\/li><li><span style=\"color: #000000;\">provides bereavement care and support to patients&#8217; carers<\/span><\/li><\/ul><h5><span style=\"color: #000000;\"><strong>Social worker<\/strong><\/span><\/h5><div data-height=\"449\"><ul><li><span style=\"color: #000000;\">assesses what sort of support patients, their family and carers need, and identifies ways they can receive this support<\/span><\/li><li><span style=\"color: #000000;\">may provide counselling and emotional support to patients, their family and carers, including working through feelings of loss and grief<\/span><\/li><li><span style=\"color: #000000;\">assists with communication within the family (including any relationship issues) and with other health care professionals, including any changes to patient&#8217;s care goals<\/span><\/li><li><span style=\"color: #000000;\">discusses ways of coping and how to emotionally support patient&#8217;s children, grandchildren or other dependents<\/span><\/li><li><span style=\"color: #000000;\">can help patients work out ways to record their memories<\/span><\/li><li><span style=\"color: #000000;\">provides information and referrals for legal matters, financial support, home respite care, meal services, personal alarms and aged care services<\/span><\/li><li><span style=\"color: #000000;\">may help people with limited support from family or friends to arrange temporary or permanent care for dependents\u00a0<\/span><\/li><\/ul><h5><span style=\"color: #000000;\"><strong>Occupational therapist or physiotherapist<\/strong><\/span><\/h5><div class=\"cancer-content not-truncate-text\" data-height=\"480\"><div class=\"cancer-content-inner\"><ul><li><span style=\"color: #000000;\">helps patients to manage the physical aspects of their daily activities, such as walking, bathing, and getting into and out of bed and chairs safely<\/span><\/li><li><span style=\"color: #000000;\">advises patients on physical aids to improve their mobility<\/span><\/li><li><span style=\"color: #000000;\">teaches carers and family the best ways to move the patients or help them sit and stand<\/span><\/li><li><span style=\"color: #000000;\">assists with pain relief techniques, such as positioning patients&#8217; body in a better way, using hot and cold packs, and stimulating certain nerves in their body<\/span><\/li><li><span style=\"color: #000000;\">shows patients how to exercise to reduce pain and stiffness and increase mobility and energy<\/span><\/li><li><span style=\"color: #000000;\">uses physical therapy to help clear congestion from patients&#8217;
