Objectives for audience
1. Understand the concept of transitions of care
2. Understand concept of continuity of care and measures which support it
3. Understand the landscape of oncology, palliative and primary care in Sri Lanka
4. Factors which can improve patient experience and outcomes during transitions
1) Understand the concept of transitions of care
- Transitions of care can occur:
– Between settings; e.g., hospital to home
– Across health states; e.g., curative care to palliative care
– Between providers; e.g., specialist to general practitioner
- Challenges/Risks during transitions
2) Understand concept of continuity of care and measures which support it
- In terms of relational, informational and management continuity
3) Understand the landscape of oncology, palliative and primary care in Sri Lanka
- Provide an overview of hospital, community and carer services ( if audience is only from a particular region , to give relevant information that region). Services are likely not uniform over different regions of Sri Lanka.
- Provide information on potential limitation of medications which may be needed eg. opioids, injection medications
4) Factors which can improve patient experience and outcomes during transitions
- Use local case scenarios to illustrate points 1 to 4
